Functional Appliances in Orthodontics: Understanding Mandibular Growth

Since the early 20th century, removable functional appliances have played a crucial role in orthodontic treatment. These appliances are designed to influence the growth of the mandible (lower jaw) by altering its posture. They achieve this by applying forces that stretch muscles and soft tissues, resulting in a shift in the neuromuscular environment. This, in turn, triggers bone remodeling and growth modification of the mandible.

However, the extent of change achieved with functional appliances remains a topic of ongoing research and debate. Numerous studies have investigated their impact on mandibular growth. Animal studies have indicated that these appliances stimulate cell proliferation within the condylar cartilage and promote bone remodeling at the anterior border of the glenoid fossa.

While promising, the direct application of these findings to humans is complex due to differences in growth patterns, morphology, and physiology between species. Over the past two decades, clinical trials involving humans have suggested that functional appliance therapy can lead to a modest but noticeable increase in mandibular growth.

Research indicates that functional appliances are most effective in cases with:

  • Overjet up to 11 mm
  • Increased overbite
  • Active facial growth
  • Good patient cooperation

The mechanisms by which functional appliances influence mandibular growth are still debated. Some experts believe they primarily increase mandibular length through ramus growth, while others suggest they primarily reposition teeth. It's important to note that treatment effects can also include condyle relocation, an increase in the vertical dimension, and changes in incisor inclination to correct sagittal discrepancies.

In this course, you will gain the knowledge and skills to:

  • Diagnose Class II malocclusion effectively.
  • Identify and manage different types of Class II malocclusion using various treatment modalities.
  • Understand the role of headgear and functional appliances, particularly twinblock appliances, in achieving optimal orthodontic outcomes.
