This course focuses on caregiver education and the practical application of an evidence-based caregiver assessment. We delve into relevant literature including a needs assessment and critically appraised topics to highlight the critical importance of pre-discharge education for individuals returning home from inpatient rehabilitation hospitals.

Guided by the Canadian Model of Occupational Performance (CMOP) and the Person-Environment-Occupation Performance (PEOP) model, this doctoral capstone project emphasizes program development to improve caregiver outcomes.

The core of this course is the Preparedness Assessment for the Transition Home after Stroke (PATH-s). This valuable caregiver assessment tool identifies gaps in caregiver education, ensuring both patients and caregivers are equipped for a successful transition back home. While initially developed for stroke patients, the PATH-s has proven adaptable and effective across a range of patient populations within the inpatient hospital setting.

Addressing identified caregiver education gaps involves occupation-based interventions tailored to each item within the PATH-s. You'll gain access to practical resources including helpful handouts designed to support daily occupations and mitigate caregiver burnout.

Data demonstrates the effectiveness of implementation and increased caregiver comfortability approximately 10 days following patient discharge.
